(3)(d) The secondary upright large freezer was not locked located in the pathway entering the playroom (3)(f) The program furniture was not securely anchored in the play room. (5)(a) All electrical outlets throughout the program space were not tamper-resistant. (5)(b) The two outlets in the bathroom and the kitchen sink were not equipped with a ground fault circuit interrupter(GFCI) outlet type. (5)(e) The extension cord behind the fish tank was not on a powers strip with a surge protector.
